You slog through USMLEs, collect letters of recommendation and pray for a residency position. But what if you don’t match? Typically, that meant your U.S. dream was dead in the water.
And quietly, Rhode Island is becoming a sanctuary for IMGs who can’t or don’t want to follow the standard residency route. The state provides a license for foreign, experienced physicians to practice medicine with supervision, without requiring them to undergo a U.S. residency first. It is known as the Limited International Physician License. This avenue is not a loophole, it’s a legislative way to address urgent physician shortages while allowing qualified IMGs to emerge from the shadows and into the clinic.
What does it mean to have a “limited licence”, though? Think of it as a bridge. It’s not a complete, independent licence, but it is probably the only legal way to practise medicine in the United States without having a residency behind your name.
In Rhode Island, this isn’t just policy; it’s written into state law. The provision allows the Board of Medical Licensure to issue a “limited registration” to international physician graduates. In other words, it enables an IMG to practise only in a certain hospital or healthcare system that is willing to sponsor them.
Rhode Island is saying, “If you’re an experienced IMG, ECFMG-certified, and a hospital or healthcare facility hires you full-time, you may work under a limited licence, prove your competency, pass Step 3, and then move directly to full independent practice.”
This law became effective June 23, 2025.
Supervised Practice: You practise under supervised conditions, with multiple licensed physicians on staff. You get a temporary licence that is generally granted for a year and is renewable.
The purpose is to allow the facility time to assess your clinical skills. Upon meeting certain conditions (such as passing Step 3 and finishing the supervised period), you can then apply for a complete, unlimited licence.

What is your day-to-day like with a limited licence? It’s good to be aware of the guidelines so you know what to expect.
You can provide medical care only instead of the sponsoring hospital or clinic. Assist in patient care under the guidance of attending physicians.
