Let’s be honest, if you’re an International Medical Graduate (IMG), you know exactly what to do. You’ve passed the USMLEs, you’ve got years of experience under your belt, and yet you find yourself staring at the same bottleneck that leaves thousands of talented doctors in limbo every year:
The US residency match is frustrating and expensive and, quite frankly, seems like a denial of the experience you already have.
But what if I told you that the script is getting rewritten? Welcome to Wisconsin. In an audacious attempt to combat a dire physician shortage, particularly in its rural outposts, Wisconsin just overhauled the rules. A new reform allows seasoned IMGs to skip the residency grind altogether and go straight to work. That’s not a loophole, it’s a legal, structured pathway.
In this guide, we will walk you through how to qualify for a Wisconsin provisional license, where you can go to work, what earnings look like and how you can take the next step towards US medical practice without having to repeat residency.
Wisconsin has a problem. About 30 per cent of the country’s population lives in lovely rural towns, yet only about 10 percent of doctors practice there. Hospitals are closing or struggling to stay open, and patients are travelling hours for care. The state is projected to be short over 2,200 physicians by 2030.
In 2024, Wisconsin enacted a law establishing the provisional license for internationally trained physicians. The reasoning is simple, Why bend a doctor with 10 years of training in his homeland to begin again as an intern? Wisconsin joins a progressive wave of other states saying, “We need you now”. They are betting your experience will be the cure for their healthcare desert.
Wisconsin is saying:
“If you’re an experienced IMG, already trained and licensed abroad, and a Wisconsin healthcare facility hires you full-time, you can work under supervision for 3 years, and then convert directly to a full Wisconsin medical license.”
Well, how does this pathway actually work? On July 1, 2024, the Wisconsin Medical Examining Board may issue a Provisional License to IMGs who meet qualifications. Consider it your “get-in-the-door” card.
Get Hired: This license is not something you can attain on your own. You must have a written job offer from an approved employer, including hospitals, Federally Qualified Health Centres (FQHCs), Community Health Centres (CHC) or Ambulatory Surgical Centres.
Supervised Practice: After obtaining licensure, you practice medicine under the supervision of a fully licensed physician. You’re working and flying solo, but not alone yet.
You can convert your provisional license to a full, unrestricted one and practice throughout the state after (3) years adhering to good-standing guidelines.
This isn’t an “exam-only” gimmick. It is based on employment and asks for your previous skills above the training given.
Let’s look under the hood. The legal power that underlies this opportunity is Wisconsin Assembly Bill 954 (AB 954), which was chaptered into law in March 2024. It allows the state medical board to review foreign credentials through the lens of whether they are “substantially similar”.
This is important: It’s a regulated, transparent process. The board is not “lowering standards”; they are acknowledging that training done abroad can be as valid as training done in the United States. This framework is specifically designed to ensure patient safety, and the supervisory period is introduced while extending the workforce. It’s a win-win, with bipartisan support to address an urgent infrastructure need.
This pathway is a golden ticket, but you must fulfil certain criteria. It is for seasoned attendings, not new graduates. You can read below our checklist according to the provisions of the law:
Degree: Medicine, a school recognised by the ECFMG
Exams: Completed all stages of the USMLE including USMLE Step 1, 2 CK and 3.
Certification: One standard ECFMG certificate.
Postgraduate Training: You must have experienced a residency or postgraduate training “substantially equivalent” to a US residency.
Clinical Experience: You require a minimum of 5 years of clinical practice after your residency in your home country.
Recency of Practice: You need to have practised at least 1 year over the last 5 years, which can be continuous. They’ll want to know you’re still on your game.
Job Offer: A confirmed offer with an authorised employer in Wisconsin.
Immigration: Federal status immigration (national) and work authorisation.
If you’ve been in practice overseas for the past decade, this list doesn’t look too bad compared to going through the Match, does it?
So what is life like on a provisional license? You will be practising medicine with training wheels for a while, though you’ll also be licensed as a physician. You must work under the supervision of a designated physician. Your license is connected with your employer as well. If you leave that particular hospital or clinic, your license can be revoked unless you quickly secure approval with another employer.
To keep the board happy, you (or your employer) have to submit a statement every six months affirming that you’re still practising and that you haven’t run into any disciplinary issues. It sounds like paperwork, but it’s a small price to pay to avoid residency. Three years of this supervised practice go on, but without the drama, and when the supervision ends, you convert to full licensure: You can practice independently or transfer somewhere else.

The board can approve other sites case by case.
Money talks. So, how much can you expect to make? Depending on speciality and geographic location, salaries range from $120,000 to $200,000+ per year. That’s a bit less than a fully licensed, board-certified specialist would make, but keep in mind, this is the salary of an attending physician, not a resident.
The financial advantages are not limited to the base salary. The state is so eager to fill gaps that rural areas tend to sweeten the pot. Sign-on bonuses, loan repayment options (even for loans from back home, depending on the employer) and housing stipends might be available.” You earn a consistent US salary immediately, and gain the US clinical experience necessary to eventually sit for your board certification. It is a financially savvy way to immigrate without the “homeless” phase.
This is the best part.Wisconsin law says:
After 3 consecutive years of full-time practice in Wisconsin in good standing, your provisional license can be converted into a standard Wisconsin physician license.
So to convert, you need:

Then you may apply for a full unrestricted Wisconsin license to practice medicine and surgery.
I want to be honest with you, this is not easy for everyone. The largest hurdle is the “5-year experience” rule. If you are a fresh graduate, this door is closed for the moment. Also, even though the state gave its approval of the law, it can be hard work to actually get a job offer. For employers, it requires a commitment to navigating supervision requirements and paperwork.
Then there’s the matter of autonomy. Some of the more established doctors have trouble going from being the “boss” in their home country to being supervised here in the U.S., and initially, your license is dependent on your employer. If you quit or are terminated, your license is at risk until a new qualifying job comes along. It’s a crossing into stability, not instant independence.
