For International Medical Graduates (IMGs) like myself, the pathway should be straight forward, pass the USMLEs, gain ECFMG certification, and take on impossible odds through the Match lottery system to secure a residency position. This is the traditional pathway, and for how many it seems like they only pathway. But for the thousands of qualified IMGs who do not match, what now? So your dream of practicing medicine in the United States is over?
Not anymore. States are starting to figure out that ignoring thousands of board-certified physicians is not good for anyone, especially now, when rural communities are experiencing huge doctor shortages. That’s where limited licensure pathways are useful. These enable IMGs to provide supervised care without committing to full US residency. Iowa is one of the states at the forefront of this change, and in this post, we’ll tell you exactly how to take advantage of this groundbreaking new opportunity.
Before we get into Iowa, let’s clarify a little bit what a “limited license” really means. There are basically three different kinds of medical licenses in the US:
Full Unrestricted License: This is what you have after US residency. It gives you the opportunity to practice on your own.
Training License: This is what you use during residency. It does not let you practice externally to your program.
Limited/Conditional License: This is a compromise of sorts. It enables IMGs to work in a specific environment under supervision, regardless of whether they are not in US residency.
States are filling gaps in their health care systems with these limited licenses. Rather than allowing talented doctors to become taxi drivers while awaiting the opportunity to apply for medical positions through the Match, states like Iowa have said, “You’ve got skills, you’ve got diplomas, let’s get you out there seeing some patients, under supervision, of course, while we take steps to abate our physician shortage.” You won’t be autonomous, but you will be practising medicine.
Understanding the background of the pathway is equally important for the IMGs.
Iowa is doing more than talking about IMGs; they enacted a law to get it done. Senate File 477 introduced a formal pathway for international physicians who do not have US residency to obtain licensure, effective January 1, 2025.
The Iowa Board of Medicine is now empowered to issue these licenses, but there’s a catch and it’s actually a good one. You can’t simply hang a shingle. You must have a job offer from an Iowa health care facility that agrees to supervise you. That typically means hospitals in rural regions, community health centers, or systems that are hungry for qualified doctors. The principle here is straightforward: use the talent we have on hand to remedy the shortages we can’t shrug off.
So, how do you qualify? Though also opening the door for more experienced physicians, Iowa has established some fail-safes to maintain patient safety. Here’s what you need:
You should have a medical diploma (MD or equivalent) from an accredited and recognized foreign medical school.
You lack the required USMLE passing scores. Step 3 isn’t in every list, but completing it will make you a far better candidate. A “passing score” must be achieved on the USMLE examinations.
You need to be assessed by the Educational Commission for Foreign Medical Graduates (ECFMG) and possess a valid certificate.
Here’s the big one: You have to have done a residency in your home country AND practised as a licensed doctor for five years after that training.
A formal job offer from an Iowa healthcare facility that is willing to employ you
You have to practice under board-approved supervising physicians. That is not a suggestion; that is the law.
You should pass a criminal background check, demonstrate you are in good standing with the medical board in your country of origin and have legal immigration status to practice medicine in the US.
What does this job really entail, anyway? Day one in your practice, you’re not the attending surgeon. These roles are meant to maximize your potential, all the while keeping you under a safety umbrella.
You might serve in this role as a hospital clinical associate, a rural physician extender or a supervised clinic physician, contingent upon the employer and supervising physician. You might work in outpatient clinics, community health centers, or rural hospitals. Consider it a long-term, paid “fellowship” so that you can employ your medical knowledge while getting to learn the US system. What you are allowed to do will be determined by your employer, supervisor, and hospital credentialing committee.
Under Iowa’s provisional licensure pathway (Chapter 148J), IMGs can work for an Iowa “health care facility” that agrees to hire and sponsor them.
These hospitals often need physicians in internal medicine, family medicine, hospital medicine, and outpatient specialties.
These facilities serve smaller communities and are often more open to physician recruitment.
These centers focus heavily on primary care, preventive medicine, chronic disease management, and underserved populations.
These offer structured outpatient practice with specialist collaboration.
These settings may employ physicians in geriatrics, rehabilitation, and chronic care management.
Let’s talk money. And you won’t be making attending-level money right away, but this is not an unpaid observership. You are doing work, and you are being paid for it.
Plus, it’s typically around $70,000 to $120,000 per year. This can vary quite a bit depending on location (rural areas may pay more to incentivise you), the healthcare system, and your specific responsibilities.
In addition to the paycheck, you typically receive perks like health insurance, malpractice coverage and sometimes even relocation assistance. However, the financial side is not even the most important aspect as the US Clinical Experience (USCE). You’ll gain practical exposure to electronic medical records (EMRs), US workflows, and patient communication. And as icing on the cake, if you eventually want to apply for residency later, this experience is pure gold for your resume and the networking of US physicians who can vouch for you.
Start Practising Now: Rather than sitting at home counting the days toward another Match cycle, you are out there being useful, using your skills and seeing patients. Just for that emotional boost, it’s worth it.”
Real US Clinical Experience: No shadowing, you are working. This experience with the US healthcare system is quite valuable.
Beef Up Your CV: If you are still interested in a residency (perhaps even to become board-certified), this experience will put the rest of the applications to shame. You will have strong US letters of recommendation and documented clinical experience.
And you can also be financially secure. It relieves the financial burden.
We have to keep it real. This route isn’t a magic wand, however, and it comes with hurdles.
The biggest challenge is that there are not enough positions. Hospitals have to agree to employ and supervise you, and not all of them are configured to do so. You also require a sponsoring employer, and your scope of practice will be limited. You are in a supervisory position, which can come as an adjustment for veteran doctors.
Also, keep in mind that this is state-specific. That license does not automatically transfer if you leave Iowa. And although you are practising, this pathway is absolutely not a replacement for an ACGME residency if board certification is your final destination. It’s not the only door to a new life in America or, of course, medicine itself, but it’s one of the most hopeful.
