The USMLE journey is a marathon across a desert for many IMGs: years of exams, increasing financial pressure and in many cases no income at all while awaiting the residency match. To this day you are fed up because you have a good grounding and the willingness to work, but the system is holding you back. What most IMGs do not know, however, is that there are other routes which can get you to start working and gathering clinical experience sooner. One of those hidden gems is Tennessee! In the US, if you take the right pathway, you can practise medicine without residency. We will explain on this blog how this works, who is eligible for it and the process involved and provide you with some of the biggest traps to be avoided.
The Tennessee Limited License Pathway is a type of restricted medical licence limited to International Medical Graduates, physicians who have completed their education outside the United States. A temporary bridge that allows you to practise medicine in Tennessee, supervised by a licensed physician and preferably inside designated residency settings like clinics, hospitals or underserved community practices.
Let us be clear here that this is not a full medical licence and does not have the same privileges or autonomy. However, what it is is an incredibly useful transitional experience that a lot of IMGs don’t take advantage of. You’re not just waiting on the sidelines while you work toward full licensure. You earn money, gain meaningful US clinical experience, and bolster that resume residency programmes and licensing boards desire to see. Finding that combination is harder than one might think for IMGs already wrestling with a complex system.
So why Tennessee specifically? Tennessee is actually a very IMG-friendly state as compared to many others, and that’s not just a marketing language. One of the top attractions is that if you have not yet completed a US residency but want to practise in their state, they will let you apply for a limited licence right away, which eliminates one of the most frustrating hurdles IMGs often have to face. That opens the door to real, hands-on clinical experience in an actual US healthcare setting, the kind that looks strong on a residency application and, more importantly, makes you a better-prepared candidate. Tennessee has also published data on physician shortages in rural and underserved areas, indicating a true need for medical providers ready to serve there. For IMGs, that’s not merely an opportunity, but it is a small taste of what could be the best result, which many states are currently unwilling to offer.
In Tennessee, IMGs are regulated through various formal licensing processes but also some practical processes that behave like employer-based licensure pathways for limited or graduate physician-type positions. Here’s a clear breakdown:
You will need to graduate from a medical school that is referred to in an acceptable worldwide directory (or equal). Tennessee medication applicants can be appropriate clinical graduates before any licensure chances.
Generally, ECFMG certification is a requirement for many pathways, especially on transition to full licensure. But the board will look at applicants on a case-by-case basis based on the particular licence category (limited/training, etc.) and not just require it up front.
Everyone who completes and passes the USMLE Steps is much more effectively competitive. If you want to get fully licensed, longer steps are ahead; for limited or training licences, the licensure process may be somewhat flexible, but performing well on the exam obviously makes you a lot more competitive and credible as a future employee.
Any criminal past, malpractice problems, licence refusals or disciplinary issues need to be reported. Therefore, you need to have an unblemished professional document which caters to your approval and, afterwards, conversion to complete licensure.
Tennessee also specifies that a collaborating/supervising physician is required, as well as an approved practice enclosure. If you do not have a job offer nor a supervising physician, then you cannot practise regardless of what other criteria you meet.

What you won’t have is full clinical independence, and it’s worth being upfront about that. Your scope of practice is directly tied to your supervising physician, meaning what you can and can’t do will vary depending on who you’re working under and what they’re comfortable delegating. That’s not necessarily a drawback, as a good supervising physician can actually accelerate your learning. It’s better to go in with realistic expectations; it makes the whole experience work a lot better for everyone involved.
Firstly complete your core exams. Typically, most IMGs should have completed Step 1 and Step 2 CK prior to applying for opportunities. These reflect elementary knowledge of science and clinical skill. Step 3 is a huge bonus (not always required) for limited or supervised roles; this suggests your readiness and ability to make independent decisions and also makes you much more marketable now as an applicant seeking an employer to sponsor you. Step 3 may prove to be the winning detail in competitive scenarios.
Organise your documents ahead of time. Your medical degree, ECFMG certification (if applicable), updated CV with US clinical experience and strong letters of recommendation from ideally a US physician. Documenting things nicely and in an organised way reduces the time to hire for a new team member by a few weeks sometimes.
This is the hardest step of all! An essential part of the process is that you have to find a clinic or physician who agrees to sponsor and supervise you. Apply to the underserved or rural spots and network through mentors, observerships, and LinkedIn.
After receiving an offer, file your request (with some fees!) and proceed with a background check. In general, your employer will be part of this journey.
Once approved, you will start practising in a limited scope of practice, but under the supervision & guidance of your collaborating physician.
Salaries under a limited licence typically range from around $50,000 to $100,000 per year, and yes, that’s a wide range for a reason. Location plays a big role; rural and underserved areas often pay more to attract providers. Your employer, setting, and level of experience matter too. Is it less than what a fully licensed attending earns? Absolutely. But here’s the honest perspective: if you’ve been out of clinical work, stuck in the licensing limbo most IMGs know too well, this isn’t a step down. It’s a significant step up from zero income while you’re actively building toward something bigger.
Like anything worth doing, this pathway has real upsides and honest drawbacks, and you deserve to know both.
On the positive side, you’re earning income without needing to complete a US residency first.
gaining legitimate clinical experience that strengthens future applications,
and building the kind of professional network that quietly opens doors later.
Those three things together are genuinely hard to replicate any other way.
Now, the flip side is your scope of practice is limited,
and you’re largely dependent on your supervising physician for how meaningful that experience actually becomes.
The licence isn’t recognised across other states, so your options stay geographically tied to Tennessee. And perhaps most importantly, this is a transitional step, not a destination.
It doesn’t replace residency, and treating it like a long-term substitute will only delay the career you’re actually working toward. Go in clear-eyed, and it’s a smart move.
If you want to make this pathway actually work for you, strategy matters more than most IMGs realise. Start by prioritising USMLE Step 3 early, as it significantly strengthens your application and signals readiness to employers.
When targeting positions, lean toward rural and underserved areas; competition is lower there, and opportunities are more genuine.
Every clinical hour you log is USCE, so treat it that way and build your CV intentionally. Network consistently on LinkedIn, connect with mentors who’ve walked this road, and reach out directly to clinics.
Be flexible with location, at least initially because rigidity costs opportunities. And keep reminding yourself this is a stepping stone, not the finish line. Here’s something most people won’t tell you, though: employers at these settings often care more about reliability, communication, and a collaborative attitude than board scores alone. Show up, be consistent, and that reputation will travel further than any number on a transcript.
Working on a limited or supervised licence in Tennessee is not the dream job, but it is what you want to do to get there. The majority of IMGs utilise this period to enhance their residency applications with authentic U.S. clinical experience, getting connected with physicians, and receive higher quality letter(s) of recommendation. The best part? The ability to work and make money while applying for the Match alleviates some of this financial burden. It bears clarifying, however, this pathway does not substitute for residency. Residency is important for you if your objective is total independent practice in the U.S. This route, however, gives you the relevant boost and makes applications confidently competitive.
